Lednum Transitional Recovery Program (LTRP)
Since 2021, RHD’s Community Transitional Recovery Program (CTRP) has been helping individuals with high and complex needs who are exiting institutions or other acute crisis settings to further prepare them for community-based living and planning for community inclusion.
In response to an increase in demand, we are pleased to offer the Lednum Transitional Recovery Program (LTRP). We are bringing the same innovative approach utilized at CTRP to community members who need more intensive support than what is offered at our other programs.
LTRP employs an interdisciplinary team consisting of a program manager, case managers, and a part-time nurse. The program is staffed from 7 am to 11 pm, seven days a week. From 11 pm to 7 am, each apartment utilizes remote monitoring technology with sensors (no cameras) and the ability to speak to a live person with the push of a button. LTRP provides a variety of services tailored to meet each participant’s needs and person-centered goals. The program focuses on continued recovery and further developing life skills that are essential to successfully living more independently in the community.
LTRP works with community stakeholders to link participants with community resources for transportation, psychiatric and primary care services, and other needs to ensure continuity of care after transitioning out of the program.
LTRP offers:
- Private single occupancy apartments
- A Part-Time Nurse
- A ratio of two or more staff persons to five participants between 7 am and 11 pm most days.
- Remote monitoring from 11 pm to 7 am every day.
- Training on how to use Uber and NEMT (Non-Emergency Medical Transportation) rather than relying on LTRP for transportation.
- Medication reminders and coaching, but not medication administration.
- A program length of up to nine months.
While this program is staffed 16 hours per day and seven days per week, the program is designed specifically for those who have both the ability and desire to transition to greater independence during program’s nine-month duration.
