Crossroads Community Services
Crossroads Community Services provides multiple services such as Blended Case Management, Permanent Supportive Housing, Flexible Housing Fund/Bridge program, with the intention of supporting individuals with mental illness to have the quality of life that they desire. The program envisions staff as partners to “walk beside” rather than “care for” program participants. BCM and other staff will be infused with a vision of what might be and expectation that each person’s life can improve. The purpose of the Permanent Supportive Housing Program is to assist homeless persons with mental illness in the transition from street and shelter to permanent housing and maximum self-sufficiency by achieving residential stability and increasing skill level and/or income. In order to qualify for this program, persons must call 211 and complete the assessment. We are not able to take direct referrals for this program. Blended Case Management services work with participants to link and refer them to the supports and services within the community to support them in their recovery.
Our Street 2 Feet Outreach Center provides case management and support services to the homeless population in Monroe County. There, clients can take showers, do laundry, search for jobs and housing, receive material goods to care for their hygiene, etc. Often additional supportive services are needed in order to achieve these goals such as the Flexible Housing Fund Program.
In continuing with the spirit of support and assisting individuals with mental illness to have the quality of life they desire, the Flexible Housing Fund Program provides financial assistance to maintain and acquire sustainable, affordable housing. Crossroads Community Services believes that the persons living with serious mental illness have the ability to manage their psychiatric symptoms with the adequate supports and resources. They have the ability to live in residential settings that are normalizing and non-stigmatizing and the potential to engage in useful and productive work activity. These fundamental beliefs in the capacity of individuals to enter fully into the community life are the basis of Crossroads services. The program is dedicated to providing services to anyone who is eligible within Carbon, Monroe, and Pike Counties.
