RHD South Dakota delivers person-centered case management services to people with intellectual disabilities.
At RHD, case managers work collaboratively with individuals to ensure they have equal opportunities and receive the services and supports they desire in the community of their choice. This process begins immediately upon receiving a referral, with a goal of contact with individuals within two working days of a referral to our services. After initial contact, we encourage frequent contact with everyone served. We maintain our connection with the people we serve through home and community visits, or in any setting that makes sense to the individual. Staying connected with the team has proven to produce better outcomes for the individual we support. RHD supports people as they work to be active, full members of their communities.

Planning & Linking Services and Supports
Through planning, we identify resources and connect people to those resources that are “Important To, and Important For” them. Once an initial plan is in place, we consider it to be a living document that should be updated as individuals experience new achievements, challenges, and other changes in their lives. Planning is done with an interdisciplinary team. Although the case manager is responsible for facilitating the production of the plan, the person and family served determine the direction of the plan. It is the case manager’s job, through person-centered planning, to illustrate the individual’s vision, which includes goals, needs, strengths, achievements, and preferences. Our case managers utilize Person-Centered Thinking tools to respond to an individual’s needs. In this way, case managers can best determine what supports a person wants and needs — and what skills are needed from the staff to support that person. Case managers ensure that the people we support are receiving services and support in the manner they prefer. Monitoring of paid services is completed routinely throughout the year.
Advocacy
RHD case managers will work with your team, and the surrounding community, to advocate for services and funding to meet the needs of people with developmental disabilities — as well as providing individuals with information about advocating for themselves. Our goal is to provide the highest quality of support and services by listening to the people supported and the people who know them best. We strive to promote a culture of inclusion and individualized support.
