The RHD NC Enhanced Supportive Housing Program provides a team of five staff members to ensure that persons with serious mental illness who have or are experiencing homelessness have an opportunity to live successfully in the community with supports. The team consists of a Psychiatrist, Nurse, Licensed Clinician, Vocational Qualified Professional, Support Qualified Professional, ADL Specialist and a Certified Peer Specialist. The ESH program serves Adults with Medicaid who are residents of Durham, Wake, Johnston or Cumberland Counties. Eligible consumers may have the following:
Diagnosis of a severe and persistent mental illness, Multiple Inpatient Psychiatric Hospitalizations, Recurrent Psychotic Symptoms, Experienced homelessness, Difficulty Managing Housing, and Difficulty Maintaining employment.
